Assistance Program
Our mission at the Medication Assistance Program is to get
free medications for those who meet the income and insurance
guidelines. A patient must be uninsured or unable to get a
particular medication through their insurance company. Medicare-D
patients may still qualify if they are in the doughnut hole
or if their plan refuses to cover a medication. There is also
a low income requirement.
It is necessary to bring in a proof of income. This should
be a tax return if one was filed. Other proofs of income can
be a bank statement, an employer statement, if applicable,
or a social security statement. We also need an accurate list
of medications.
We are located on the lower side of the Angel Medical Center
Home Health and Hospice Building. Our hours are from 10:00-4:00
on Tuesdays, Wednesdays, and Thursdays. Please call us at
