For all procedures, please come to Admissions / Registration
area. Physical Therapy and Occupational Therapy patient’s
need to go directly to Physical Therapy to register.
Registration is open Monday through Friday from 7am
to 4:30pm.
Please register at Emergency Registration before 7am
or after 4:30pm and on weekends.
Please arrive at least 15 minutes before exam time to
register.

We must have an order from your physician for all procedures.
All surgery patients should pre-register at least one
day prior to procedure.
Report to Admissions/Registration to pre-register.
Please bring your Patient
Information Sheet when you come for your procedure.
